10 October 2018

What is Canadian Patient Safety Week?

Canadian Patient Safety Week is a national annual campaign started in 2005 to inspire extraordinary improvement in patient safety and quality. Working together, thousands of healthcare professionals, patients, and families help spread the message to Ask. Listen. Talk. Healthcare facilities and health organizations across Canada participate in the week by holding events and activities to increase awareness of patient safety in their local area.

What is the theme for Canadian Patient Safety Week?

The theme for Canadian Patient Safety Week 2018 is Not All Meds Get Along, prompting patients and healthcare professionals to initiate medication reviews for any patient on 5 or more medications. Here are the 5 life-saving questions to ask about your medications:

Changes?  Have any of my medications changed? Why?

Continue? What medications should I continue taking? Why?

Proper Use? What is the proper way to take my medications?

Monitor? What side effects should I monitor for?

Follow -Up? When should I come in for a Follow-Up?
